How to Fill Out a Membership Card
Please read the following information. You will find the link to the membership card signing form at the bottom of this page.
There are NO membership dues until we sign a contract with the State and we actually begin receiving payment from the State for appointments we've bargained for.
You can sign a membership card and no money will be deducted for dues.
This card helps us contact you with information about the Union and bargaining while maintaining confidentiality and ensuring that no agency owners or staff have access to members' information.
You must be a member who has signed a card to vote on the contract.
To fill out a card:
Only the boxes in the form that are highlighted in yellow need to be filled out. All the other boxes are optional and/or don't apply to us. Your Cell number and your Home Phone can be the same.
There will be a second page after you've filled out your information, which is called "AUTHORIZATION FOR VOLUNTARY PAYROLL DEDUCTION AS A CONTRIBUTION TO PEOPLE". This does not apply to us, is not related to dues, and you are not required to provide a contribution.
